ECG paper is a visual time scale as well as a grid. Counting horizontal boxes is only useful when the paper speed is known, because the same number of boxes represents a different duration at 25 mm/s and 50 mm/s. WorldCalculate turns that grid relationship into a small, auditable conversion tool. It reports the time of one small box, one large box, and the entered combination without attempting to interpret the waveform.
On the horizontal axis, the paper moves at a stated speed in millimetres per second. A one-millimetre small box therefore represents the time needed for one millimetre of paper to pass. At 25 mm/s, that is 1/25 second, or 0.04 second. At 50 mm/s, it is 1/50 second, or 0.02 second.
Five small boxes make one large box in the standard grid. The tool preserves that relationship rather than asking a reader to remember a single number that may be wrong for a faster tracing.
Small-box time is 1 ÷ paper speed because one small box is one millimetre wide. Large-box time is five times the small-box time. Total time is the small-box count multiplied by small-box time plus the large-box count multiplied by large-box time.
For example, at 25 mm/s, five small boxes represent 5 × 0.04 = 0.20 seconds. One additional large box represents 0.20 seconds, so the combined entry is 0.40 seconds or 400 milliseconds.
A tracing printed at 50 mm/s spreads the same electrical event across twice as many millimetres as at 25 mm/s. If a reader assumes the standard speed without checking the header, every interval calculated from the grid can be wrong by a factor of two.
Look for the speed printed on the strip, displayed by the ECG machine, or recorded in the report. Do not infer it from the appearance of the paper alone, especially when a digital export has been resized.
The calculator allows both counts so a learner can enter the way an interval was measured. If a measurement is already expressed entirely in small boxes, enter zero large boxes. If it is described as two large boxes and one small box, enter those counts separately.
Because a large box already contains five small boxes, do not count the same physical boxes in both fields. For one large box, enter one large box and zero additional small boxes, or enter five small boxes and zero large boxes, but not both versions together.
At 25 mm/s, one large box is 0.20 seconds. Three large boxes are therefore 0.60 seconds. At 50 mm/s, three large boxes are 0.30 seconds. The box count did not change; the paper speed did.
For a mixed count of two large boxes and three small boxes at 25 mm/s, the time is (2 × 0.20) + (3 × 0.04) = 0.52 seconds. The result panel also shows 520 milliseconds so the unit is easy to carry into notes.
The horizontal axis describes time. The vertical axis describes voltage and depends on a separate calibration, often expressed in millimetres per millivolt. This tool intentionally ignores vertical boxes so a reader does not confuse a voltage measurement with an interval measurement.
If the question concerns wave amplitude, gain, calibration pulses, or a digital signal scale, use the relevant tracing information and a separate method. A correct horizontal conversion cannot validate a vertical measurement.
The most common mistake is using 0.04 seconds for every tracing. That is appropriate for one small box at 25 mm/s, not at every speed. Another mistake is reading a resized screenshot whose pixels no longer correspond to printed millimetres.
A third mistake is counting a border or grid line inconsistently. State whether the interval begins and ends at the selected waveform landmarks, then count the horizontal distance between them. The calculator handles the arithmetic after the measurement has been made.
The output can tell you how much time the entered horizontal distance represents under the selected paper speed. It cannot tell you whether a PR, QRS, QT, or RR interval is normal, nor can it distinguish a wave from artifact.
Clinical interpretation requires the complete tracing, lead, calibration, rhythm context, symptoms, history, and professional training. Never use a box conversion alone to diagnose an emergency or delay urgent care.

Can I measure an image? Only if the image preserves the grid and the paper speed is known; the tool does not perform image measurement.
Why does the result show millimetres? It is the horizontal paper distance represented by the entered count, not a voltage measurement. Why include both counts? To keep the user's measurement language visible and prevent hidden conversions.
Record the paper speed beside any measured ECG interval. Keep the original tracing or a calibrated export available, and note the waveform landmarks that defined the measurement. If the count is uncertain, label it as an estimate rather than manufacturing precision.
