Formula and methodology
Formula: HEART = history points + ECG points + age points, where age <45 = 0, 45–64 = 1, and ≥65 = 2, + risk-factor points + troponin points. Maximum 10.
This page accepts the scored history, ECG, risk-factor, and troponin components and derives age points automatically. It is an emergency-care risk-stratification worksheet, not a home chest-pain rule-out test or a substitute for serial assessment.

Assumptions and limits
- The person is an adult being assessed for possible acute coronary syndrome in an appropriate clinical setting.
- History and ECG points have been assigned by a qualified professional using the approved HEART convention.
- Troponin points use the assay’s own upper reference limit and the documented laboratory result.
- Age points follow the displayed cutoffs of under 45, 45–64, and 65 or older.
- Risk-factor points follow the selected version’s definitions and the documented medical history.
- The score does not replace serial ECGs, serial troponins, examination, imaging, or an emergency pathway.
- A low score does not make dangerous symptoms safe to ignore and a high score does not identify a single diagnosis.
