Formula and methodology
Formula: Classic Centor = fever + absent cough + tonsillar finding + tender anterior cervical nodes. Modified McIsaac adds +1 for age 3–14, 0 for age 15–44, and −1 for age 45 or older.
This page adds documented sore-throat findings and makes the age adjustment visible when the modified model is selected. It is a screening arithmetic aid, not a diagnosis of group A streptococcal infection or an automatic treatment decision.
This result follows the calculator's declared inputs, precision, validation boundaries, and model limits.
Input contract
- Scoring model — 2 choices
- Age — years; minimum 3; maximum 120
- Fever documented above 38 °C — 2 choices
- Cough absent — 2 choices
- Tonsillar swelling or exudate documented — 2 choices
- Tender anterior cervical nodes documented — 2 choices
Worked example
| Input | Value |
|---|---|
| Scoring model | mcIsaac |
| Age | 30 |
| Fever documented above 38 °C | yes |
| Cough absent | yes |
| Tonsillar swelling or exudate documented | no |
| Tender anterior cervical nodes documented | no |
McIsaac score = 1 + 1 + 0 + 0 + 0 age adjustment = 2 points.
Assumptions and limits
- The findings were assessed and documented by an appropriate clinician or approved pathway.
- Fever means a documented temperature above 38 °C in the scoring convention used here.
- The tonsillar field represents the documented swelling or exudate component, not a guess from throat discomfort alone.
- The modified model applies the displayed age bands and the classic model does not add an age point.
- The score is intended for the clinical population and pathway in which the selected model has been validated.
- A score does not replace rapid testing, culture, examination, differential diagnosis, or local antimicrobial guidance.
- The calculator does not assess airway compromise, peritonsillar infection, dehydration, or other urgent causes of sore throat.
- Children younger than the displayed lower bound and people with rapidly worsening symptoms need professional assessment rather than extrapolation.
